At Mercy Health- St. Vincent Medical Center (MHSVMC), the PGY2 Ambulatory Care pharmacy resident will be given opportunity to develop clinical expertise through the Medication Management Service. By working within a patient centered medical home (PCMH) model, the resident will be able to provide patient care that is structured, delivered, and coordinated around the specific needs of each patient. By maximizing medication benefits, the Medication Management Service is part of the healthcare team working to improve outcomes related to chronic diseases with patient visits in provider offices, hospital, pharmacy, home and by telephone. Throughout the year the resident will take an active role working with collaborative practice agreements, precepting student activities, completing a research project, committee involvement, and learning about Lean Six Sigma principles.
