Department Description The Pharmacy Department provides comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps and teaching) throughout the Medical Center complex 24 hours per day. The Department is comprised of four inpatient, six outpatient pharmacies, multiple specialty clinics, two state of the art infusion centers, a sterile production lab, and the full complement of pharmacy purchasing, stores and distribution. THIS IS NOT AN H1- B OPPORTUNITY ResponsibilitiesThis position will work in Specialty Clinics, participating in the process of developing new pharmacy ambulatory care Specialty services and direct patient care activities, which include teaching classes to patients and other health care professionals. The Staff Pharmacist II works with a technician III to review charts and make recommendations. Provides and documents enhanced clinical services to patients within the Specialty Clinic and Ambulatory care in collaboration with physicians to improve patient outcomes and clinic revenue. Provides pharmaceutical care to ambulatory patients, Orders lab work for patients, Interprets Laboratory results and recommends therapy based on Clinic protocols. Works in a team environment with other pharmacists, technicians, residents and nurses. Participates in the Quality Assurance for clinic activities. Creates assessments of research and publishes results, and aids in development of research for areas related to ambulatory care, specialty pharmacy, etc. Provides patient-specific analysis and consultations (oral or written) when appropriately requested. The Staff Pharmacist II will work in the Specialty Clinics to provide services to patients and assist patients in getting their medications. The pharmacist's responsibility is to monitor drug therapy to achieve desired outcomes and to avoid drug toxicity and the dosing of medication based on patient demographics, available treatment guidelines and response. The pharmacist works as an integral part of the patient care team and assists physicians and nurses in providing drug therapy. Required Qualifications
California Pharmacist License.
Doctor of Pharmacy Degree from an accredited college or university or equivalent experience.
Computer skills that facilitate Clinical patient data medication order entry and other basic clinical software programs.
Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English.
Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA).
Experience with navigating third party reimbursement programs.
Completion of a Pharmacy Practice PGY1 and/or PGY2 Residency or equivalent experience.
BPS or CSP Certification.
Experience precepting, and evaluating residents and students.
Experience working with patients in ambulatory care clinic setting.
