This residency offers a unique opportunity to bridge practice between a large family medicine clinic and a community pharmacy. The resident will be developing clinical skills through direct patient care experiences at Bethesda Family Medicine Clinic in St. Paul and a Walgreens community pharmacy. These sites work in collaboration to achieve a common goal: to develop a continuum of care practice model that can be replicated and utilized by health care organizations. Such work will improve patient health care experiences, reduce hospital readmissions, increase patient medication adherence, improve patient outcomes, and decrease overall health care spending. Bethesda Family Medicine clinic is a University of Minnesota Physicians (UMP) clinic that is the longitudinal site for the University of Minnesota St. Joseph’s Family Medicine Residency Program. This interprofessional primary care provides care to patients of all ages where the clinical pharmacists are an integral part of patient care.
The specific program for this resident may vary in structure based upon the resident's entry-level knowledge base, skill and interest. The resident will receive training and develop competence in managing and improving the medication-use process, providing evidence-based, patient-centered medication therapy management with interdisciplinary teams, exercise leadership and practice-management skills, demonstrate project-management skills, and provide medication and practice-related education/training.
This residency is designed to produce a practitioner that is a leader in health care change and exceedingly competent in their abilities to facilitate similar programs in the future.
The resident will participate in the provision of patient-care services through the following:
Direct patient care delivered both in the primary care clinic and community pharmacy settings
Participation in comprehensive medication management programs with a variety of disease states and patient populations
Involvement in a collaborative practice through Collaborative Practice Agreement
Transitional Care Management
Panel management/Management of Patient Populations
Community-based immunization delivery
Community outreach programs
Health and wellness programs
The resident will participate in the following activities that strengthen practice management/leadership and teaching skills:
Provide experiential teaching in a clinical practice environment and serve as a mentor to pharmacy students
Provide didactic lectures to family medicine resident physicians
Facilitate weekly topic discussions with pharmacy students
Actively participate in professional pharmacy organizations activities and attend at least one national pharmacy conference during the residency year
Opportunities to teach at the University of Minnesota College of Pharmacy
Design and conduct a practice-based research project
Execute a marketing strategy for the promotion of patient care services
Participate in program-wide monthly “Academic Day” events
Completion of online coursework related to practice and/or teaching topics

Residents are eligible for Paid Time Off (PTO). The rate at which you accrue your PTO will be based on your years of service, average hours per week, and position. Resident pharmacists may accrue approximately 10 days depending on average hours worked and time in position
