Tabula Rasa HealthCare (TRHC) is a family of companies that focuses on medication risk mitigation, pharmacy cost management, Medicare risk adjustment, and leveraging technology to improve healthcare (www.tabularasahealthcare.com). One of its companies, CareKinesis, is the first and largest pharmacy that provides personalized medication therapy management, dispensing, and technology solutions exclusively for Program of All-inclusive Care for the Elderly (PACE) programs throughout the United States. These programs provide comprehensive, long-term services and support to older adults (> 55 years of age) that qualify for nursing home care but are able to reside in the community through PACE. CareKinesis specifically focuses on mitigating medication safety, improving medication non-adherence, and reducing participant hospitalizations. It accomplishes this through consultations and communications with board-certified geriatric pharmacists, utilization of medication risk mitigation technologies and pharmacogenomics services, an electronic medication management platform, custom adherence medication packaging, and flexible medication access options.
PGY1 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists who are responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. The goal/mission of the TRHC residency program is to train pharmacists by bringing together cutting-edge clinical data, research and technology in order to tailor medication regimens for individual patients from diverse backgrounds, as well as to provide guidance in their future professional endeavors.
The PGY1 pharmacy resident will gain personalized pharmacy practice experiences in a fee-for-value healthcare environment, including longitudinal and block experiences. Required rotations include enhanced Medication Therapy Management (EMTM), PACE Call Center, PACE onsite, Total Quality Management (TQM), Clinical Pharmacology & Pharmacogenomics, Teaching and Research & Writing. Elective rotations include Pharmacoinformatics & Healthcare Technology, Practice Management & Marketing, Client Development and Regulatory Affairs.
