KDHCD has an ASHP accredited PGY2 in Ambulatory Care with an opening for one resident in 2020-21. The program is set within an integrated health care delivery system anchored around a 448-bed community-based teaching hospital. With a complement of ancillary services and ambulatory clinics, KDHCD provides a broad array of primary and specialty care to a largely rural and medically underserved community. The Outpatient Pharmacy Department provides services to the community through our in-house retail and home infusion pharmacies, as well as several outpatient clinicis.
Seven Ambulatory Care pharmacists and two Pain pharmacists work in innovative, team-based models as credentialed members of the KDHCD Medical Staff providing pharmaceutical care in the areas of primary care, family medicine, chronic disease management, population health and pain management. A majority of the PGY2 training takes place in the Family Medicine, Chronic Disease Management and Rural Health clinics.
Kaweah Delta Family Medicine Center is the formal clinic training site for Kaweah Delta Family Medicine Residency providing primary care for an underserved population. Our clinical pharmacists provide direct patient care, patient education, and both practice-based and didactic instruction to the family medicine residents.
Kaweah Delta Chronic Disease Management Center is a referral-based, high-risk clinic for treatment and management of type-2 diabetes, COPD, tobacco cessation, heart failure, anticoagulation, pain management, and other disease states requiring resource-intensive multidisciplinary care.
Rural Health Clinics provide internal medicine, family medicine, women's health, pediatric care, and specialty services to rural areas of Tulare County.
The resident will spend the first 8 months in required block rotations which will build their foundation as a competent, independent practitioner. These include Family Medicine I and II, Pain Management, Chronic Disease Management, Scholarship and Teaching and Leadership and Practice Management. The final four months of the year are spent longitudinally between Primary Care and Electives I and II. The Primary Care rotation is a time for the resident to be the pharmacist within their own primary clinic, responsible for establishing relationships with other providers, managing their own practice, and providing comprehensive medication management. Electives will be approximately 8 week long longitudinal experiences running alongside the Primary Care rotation and will give the resident more experience in selected practice areas such as cardiology, nephrology, or transitions of care.

A rural health clinic system that includes 5 outpatient clinics gives patients in rural areas better access to health care in Tulare County. Additionally, a Chronic Disease Management Center rounds out the outpatient services focusing on improvement of outpatient self-care for patients with chronic illnesses at risk for readmission to the hospital.
