KDHCD is an ASHP-accredited PGY1 program for two residents. The pharmacy practice residency is a 12-month postgraduate currirculum that offers training opportunities in a variety of acute care and practice leadership settings. Each month the resident will rotate through a variety of patient care settings where they will have the opportunity to enhance their ability to provide safe and effective pharamaceutical care while being mentored by a clinical pharmacist in that area.
The Inpatient Pharmacy Department provides service to three campuses of KDHCD. The Department has a staff of 100+ with over 50 clinical pharmacists and operates 24/7 inpatient pharmacy services. A full array of technology is available to assist with distribution of medications to our patients. This includes automated inventory management, automated dispensing cabinets, bar code technology, automated TPN compounder and workflow management systems.
Pharmacy services utilize an integrated decentralized staffing model. Pharmacists practice on assigned nursing units participating in order verification, provision of drug information, medication history verificaiton and clinical pharmacy services. Clinical services include pharmacokinetics, anticoagulation management, and automatic therapeutic interchanges. Specialty pharmacy services include Infectious Disease, Critical Care, Emergency Medicine, Formulary Management, Medication Safety, Oncology, Pediatrics, Anemia Management, Pain Management and Anticoagulation.
Residents will rotate in 4 week blocks, completing all required rotations and selecting 5 elective rotations offered by the program. Longitudinal rotations will begin during the 3 week orientation period and continue throughout the residency program year. Residents will receive the month of December to participate in ASHP Mid-year and complete longitudinal project work. Residents will also be supported to attend California Society of Healthcare Pharmacists (CSHP) Seminar in October and Western States Residency Conference in May of the program year

A rural health clinic system that includes 5 outpatient clinics gives patients in rural areas better access to health care in Tulare County. Additionally, a Chronic Disease Management Center rounds out the outpatient services focusing on improvement of outpatient self-care for patients with chronic illnesses at risk for readmission to the hospital.
