At Stony Brook Medicine the TH Staff Associate Level 4, Boost RN Case Manager m ust have both hospital and transitional care experience. The Boost Project (Better Outcomes for Older adults through Safe Transitions) is directed at assessing and identifying adult hospitalized patients who may be at a high risk for readmission. The RN following these patients must have good clinical and communication skills to address the needs of the patient and caregiver throughout their hospital stay, with continued follow up at home or when transitioned from rehab.
Duties of a TH Staff Associate Level 4, Boost RN Case Manager in the Care Management Department may include the following but are not limited to:
Prior experience with Case Management or Utilization Management
Assesses potential new Boost patients and follows all readmitted patients in the ED or on the unit.
Understand levels of care including Inpatient, Observation, Emergency Room and Outpatient
Understand the importance of readmission prevention, LOS reduction
Knowledge of high-risk readmission criteria.
Understand insurance coverage and authorization
Effectively work with interdisciplinary team regarding transition of care including Case Managers, Social Workers, Pharmacists, Medical staff, Nursing, Physical Therapist, etc.
Active participation in rounds
Provide follow up care including appointments, authorization, medical contact, etc.
Coordinate communication with home care agencies and post-acute facilities
Knowledge of all components of BOOST including the 8P's
Understands the teach back method
Document in medical record all patient, family and follow up information, utilization of all needed resources for patients
Documents all telephonic conversations
Computer literate in Excel, Graphs, Power Point, Work, Access, etc.
Track and trend data with analysis of information.
Actively involved in performance improvement activities including program development and improvement, data collection and analysis
Adheres to all State and Federal requirement, RN license, JACHO, CMS and DOH regulations
Maintain high level customer service
Required Qualifications :
Associate Degree with Transitional Care Experience
NY RN License
Courteous, professional, focus and good listening skills
Five years acute hospital experience
Home Care or Transitional experience
Good Customer Service
Certified in Case Management, UM, Quality, Risk or MCG
Past Leadership experience
Home Care experience
