Harborview Medical Center Pharmacy has an outstanding opportunity for a FULL-TIME CLINICAL STAFF PHARMACIST (ROTATING/VARIABLE SHIFTS) to join our Pharmacy team! The Clinical Staff Pharmacist (Pharmacist 3) acts as an integral member of HMC patient care services, providing pharmacotherapeutic recommendations to primary providers, nursing, and dietitian staff to optimize patient care. Responsibilities include, but are not limited to direct patient care, drug distribution, research & education, and practice management as it pertains to the care of acute care patients. RESPONSIBILITIES:Clinical:Drug therapy monitoring, including, but not limited to:* Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies* Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed* Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations* Provide drug information and education to medical teams, nursing and dietitian staff* Provide patient counseling and education Distribution:* Develop and maintain procedures to improve the efficiency and accuracy of medication distribution and administration* Facilitate efficient medication delivery and appropriate scheduling of medicationsAcademic:* Provide primary preceptorship of PGY1 pharmacy residents and student pharmacists* Provide education to medical/surgical residents, nursing staff and medical students* Present at educational conferences to multidisciplinary staff, as requested* Develop and provide nursing in-services* Provide didactic teaching to student pharmacists at the University of Washington School of Pharmacy, as requestedResearch:* Participate in collaborative research endeavors with healthcare professionals and the schools of pharmacy, nursing and medicine* Encourage pharmacy staff and residents' understanding of and participation in practice-oriented research projects* Develop research projects in the areas of: o Pharmacokinetics and bioavailability of medications frequently used in the service patient population o Efficacy of medications specifically used in the service patient population o Patient monitoring and information systems related to the optimal care of patientsManagement:* Implement and maintain departmental policies and procedures related to the safe and effective use of medications* Routinely evaluate current updates in service specific literature* Develop and maintain drug therapy monitoring systems for patients* Establish and maintain efficient lines of communication between pharmacists, physicians, nurses and dietitians* Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors* Perform drug utilization evaluation and cost containment analyses, as needed* Create a system ensuring appropriate distribution of drug information related to investigational drug agents used in the management of patientsMiscellaneous:* Submit unique work for publication in professional journals* Attend, present at and participate in national pharmacy conferences and educational meetings
