POSITION SUMMARY: The Pharmacy Practice Resident must successfully complete all of the goals, objectives and requirements as set forth by the HCA residency program manual and HCA Human Resources Policies at Henrico Doctors’ Hospital within the standard 12-month residency training program. These overall goals include, but are not limited to, the completion of all rotations in a satisfactory manner and the completion of a major project. The resident’s primary professional commitment is to the residency program. Moonlighting is not permitted during the residency year.
Organizational relationships include being directly responsible to the Residency Program Director and the Director of Pharmacy.
SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS
Read and adhere to the current residency manual entitled Pharmacy Residency Guidelines, including residency-specific policies and procedures, received during Orientation.
Complete the goals and objectives of each rotation as determined by the resident and preceptor. Projects may often overlap and/or be of varying sizes.
Maintain documentation and self-evaluation in a timely manner within PharmAcademic.
Complete a major project culminating in a paper of publishable quality prior to the completion of the residency.
Present the results of the major project at a regional residency conference.
Deliver a minimum of six presentations, either journal clubs or case presentations, during the residency year. This must include at two journal clubs and two case presentations.
Design, research, obtain credit from an appropriate credentialing body, deliver, and provide documentation for a continuing education topic presented to market pharmacy personnel.
Actively participate in interdisciplinary committee meetings as assigned, including Pharmacy and Therapeutics Committee.
Staff every other weekend, two shifts per weekend, eight hours per shift. The staffing component is typically performed at the Forest campus of Henrico Doctors’ Hospital regardless of the location of the current rotation provided that the current rotation is local. Twenty-six weekends of staffing are required throughout the duration of the residency. Ten days are granted for paid vacation or personal leave. Leave requests must be submitted for approval by the resident’s rotation preceptor and the residency program director two weeks prior to the requested date of leave. Each resident will be required to work one of either Memorial Day or Labor Day; the other may be taken as a holiday as PTO for the resident. All residents will work either Thanksgiving and the day after, or Christmas Eve and Christmas. At least three additional staffing days will be assigned the weeks of Thanksgiving or Christmas.
The resident shall perform as a pharmacist including interpreting, compounding and dispensing medications and other pharmaceutical supplies according to physician orders issued in a lawful manner by an authorized prescriber in the state of Virginia; provide requested drug information to health care professionals and patients, participate in clinical pharmacy services, and provide pharmaceutical care to patients as delineated by the Department of Pharmacy.
Manage systems for the storage, preparation and dispensing of medicinal agents using appropriate techniques and following the health systems policies; effectively supervising technical personnel who may be involved in these processes.
Evaluate medication orders, accurately and safely, compounding medications in acceptable dosage forms, appropriately packaging and dispensing pre-manufactured dosage forms.
Maintain knowledge pertaining to facility utilization of automated dispensing processes including the use of automated dispensing units, overwrap packaging, and bar coding within the facility.
Provide education on rational medication therapy and medication procedures effectively to healthcare professionals, students, patients, and the public.
Retrieve, analyze, evaluate and interpret scientific literature in order to provide accurate, concise, applicable, and timely drug information response to healthcare professionals.
Design, implement, monitor, evaluate, and intervene in patient pharmacotherapy to ensure effective, safe and economical patient care.
Apply clinical pharmacokinetic data to design effective, safe and economical therapeutic medication regimens, collaborating with all individuals involved in the medication use process, when appropriate.
Participate in the development of at least one Medication Usage Evaluation or Drug Monograph during the year, in addition to serving on designated performance improvement and/or other specific committees designed by the Residency Director.
Take responsibility for planning, organizing, networking, completing and improving work activities through effective management of time, tasks and available personnel.
Participate in implementing goals, objectives, and policy and procedures of the pharmacy department and the facility.
Effectively manage own professional practice including time and space, professional ethics, continued education, attendance punctuality, and customer focus.
Perform other duties as assigned, including fulfilling additional pharmacy staffing hours when necessary to display competency.
Travel to complete required rotations and activities of the program.
SUCCESSFUL COMPLETION OF PHARMACY PRACTICE RESIDENCY
The resident shall complete the goals and objectives of the residency program as defined in the residency manual Pharmacy Residency Guidelines and as listed in the contract entitled PGY1 Residency Requirements. These overall goals include, but are not limited to, the completion of all rotations in a satisfactory manner and the completion of a major project. Residents must obtain “achieved for residency” status on 80% of program objectives and “satisfactory progress” on any remaining programs objectives. Objectives that must be evaluated as “achieved for residency” status by the completion of the residency include Objectives R1.3.1, R1.3.3, R1.1.3, R1.1.4, and R1.1.5. Goals can be evaluated as “achieved for residency” status by the current preceptor, residency program director at quarterly review, or residency advisory committee upon meeting.
A resident who is unable to complete all requirements within a 12-month time frame will be given six months to complete the remaining assignments and will be awarded a certificate upon completion of the assignments in a satisfactory manner. A certificate will not be rewarded if the resident fails to submit all assignments successfully after the six month period following the end of the residency training program or is currently on probation. A resident will be disciplined, up to including termination, according to the Policy and Procedure for Disciplinary Action and Dismissal from PGY1 Residency Program.
Position is required to have competence to assess, treat and/or care for the age groups indicated by check marks.
Required - Effective communication skills are required. Effective written and verbal skills are required.
Required - Experience using computer applications including word processing, spreadsheet software and pharmacy information systems.
Required - Graduate of an ACPE-accredited school of pharmacy at a college or university.
Required - Licensure by the Virginia State Board of Pharmacy by examination or reciprocity.
