Rutgers, The State University of New Jersey, is seeking a Social Worker I, for the Department of Social Services at the Rutgers Cancer Institute of New Jersey.
The primary purpose of the Social Worker I is to provide well-integrated social work services to adult patient systems from age 18 years and older, and facilitate adaptation to a cancer diagnosis.
Among the key duties of this position are the following:
Provides initial psychosocial assessment to all Cancer Institute of New Jersey patients and their support systems.
Formulates psychosocial treatment plans. This may include, but is not limited to, assistance with: advanced directives, decision making regarding treatment options, patient/family education, discharge planning.
Provides direct social work services in accordance with social work assessment, diagnosis and treatment.
Identifies lack of services for patients and assumes responsibility for developing new programs to enhance patient care; identifies gap in service delivery and develops alternative strategies and/or programs to meet patient needs.
Participates as a member of a multi-disciplinary team, which includes providing clinical care to patients, as well as participation in team meetings and patient conferences, in the inpatient and outpatient setting.
Maintains current information regarding community agency servicing oncology patients, financial/pharmaceutical assistance, insurance and reimbursement information/resources.
Maintains timely documentation that includes patient’s psychosocial assessment, ongoing progress notes, and summaries of care.
Minimum Education and Experience:
Masters’ of Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE).
Required Knowledge, Skills, and Abilities:
Must be a Licensed Social Worker (LSW) or a Licensed Clinical Social Worker (LCSW) in the State of New Jersey. If not an LCSW, individual must be eligible and granted the license within six (6) months of completion of the required 3,000 patient hours. Rutgers will provide supervision for the first 2,000 hours; the last 1,000 will be the individual's responsibility.
Must be computer literate with working knowledge of Microsoft Work, Access and Excel.
Demonstrated excellent oral and written communication skills, and the ability to interact with diverse patients, physicians, and management and community members.
Must be able to function under rapidly changing and stressful conditions.
Experience working with electronic patient/client records.
Three (3) years of experience in healthcare, primarily psychosocial oncology.
Physical Demands and Work Environment:
PHYSICALDEMANDS: Standing, sitting, walking, talking and hearing. No special vision requirements. Lifting up to 25 lbs.
New Jersey Division of Consumer Affairs, Board of Social Work Examiners, and University approved verification service and verified graduate school transcripts.
LSW must be granted LCSW within 6 months of completion of required clinical hours.
