Med-Emergency Medicine - Pennsylvania-Pittsburgh - (19005129)
The Department of Emergency Medicine is looking for a Social Worker to assist patients and their families after they suffer cardiac arrest and are discharged from the hospital.
This will include performing needs assessments with patients and families, developing a post-discharge toolkit, connecting patients to appropriate outpatient resources, and performing other follow up activities.
NOTE: The hiring range is for full-time positions and will be prorated based on 30% effort.
