A Senior Social Worker/Transplant provides medical social services to patients and their families through all phases of the transplant process. The Senior Social Worker will complete Psychosocial Assessments on all potential transplant candidates as assigned, will follow patients in the Transplant Clinic, and will provide discharge planning for patients when they are hospitalized. The Senior Social Worker will also provide information, education and referrals to community resources, and works as a member of a multidisciplinary team, attending daily rounds and meetings to discuss patients’ psychosocial support and candidacy for transplant. The ideal candidate will have strong communication, problem solving and time management skills, with the ability to work independently and document in the electronic medical record as required. This position will carry a caseload and may also assist the other Transplant Social Workers as needed. Some travel to Louisville may be required.

Master's degree and at least 1-2 years of employment experience highly preferred.
