For a complete description of our benefits for this position, please view this page.UW Medical Center Pharmacy is seeking a highly motivated FULL-TIME CLINICAL PHARMACIST (Rotating/Variable Shifts) to provide pharmaceutical care for our acute care cardiac patients. Our cardiac pharmacists provide extensive pharmaceutical care to both inpatients and outpatients, and are a vital part of the multidisciplinary patient care team. In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transition of care from the inpatient to outpatient setting, and precept pharmacy students and residents. The UW Medicine Regional Heart Center provides renowned comprehensive care for coronary artery disease, heart rhythm disturbances, advanced heart failure and adult congenital heart defects including cutting edge procedures and complex surgeries. RESPONSIBILITIES:Clinical:Drug therapy monitoring, including, but not limited to:* Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies* Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed and per collaborative practice agreement.* Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations* Provide drug information and education to medical teams, nursing and dietitian staff* Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visitEducation and Research:* Provide primary preceptorship of pharmacy residents and student pharmacists* Provide education to medical/surgical providers and students, and nursing staff* Present at educational conferences to multidisciplinary staff, as requested* Develop and provide nursing in-services* Provide didactic teaching to student pharmacists at the University of Washington School of Pharmacy, as requested* Participate in collaborative research projects with healthcare professionals, pharmacy residents and student pharmacists.* Submit unique work for publication in professional journalsPractice Management:* Implement and maintain departmental policies and procedures related to the safe and effective use of medications* Routinely evaluate current updates in service specific literature* Develop and maintain drug therapy monitoring systems for patients* Establish and maintain efficient lines of communication between pharmacists, physicians, nurses and other members of the health care team.* Participate in pharmacy and hospital committees as requested* Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors* Perform drug utilization evaluation and cost containment analyses, as neededDispensing:* Perform duties of a Pharmacist 2.
