The University of Washington Medical Center has an outstanding opportunity for a part-time (36 hours/week), temporary/hourly, MEDICAL SOCIAL WORKER 1 OR 2. Under the University of Washington Medical Center Social Work Department's Policies and Professional Social Work requirements, this Social Work Professional will support Inpatient Neurology and Med D.Duties include triaging referrals, following up on previously planned discharges, conducting assessments on patients identified as having discharge coordination needs, providing support and education to patients and families, interdisciplinary care planning, and crisis intervention. Skills required include clinical insight, an ability to be comfortable working with persons with significant medical issues, a collaborative nature, strong multitasking and organizational abilities and demonstrated customer service skills. Knowledge of health care resources and experience in hospital discharges such as Skilled Nursing Facility, Acute Rehab, Long Term Acute Care, ALF/AHF placement, Home Health and Hospice is strongly preferred. RESPONSIBILITIES:Utilizes medical record and consultation with interdisciplinary staff to screen patients for service needs, provide information regarding Social Work activity and to coordinate care as part of an interdisciplinary teamAssesses for the psychosocial and disposition planning needs of patients and when appropriate, their support systems, and develops appropriate interventions to address such needsProvides patient-centered care by treating patients and their support systems with respect and dignity, involving them in their care whenever possibleServes as liaison between medical staff and patients/families, providing support and advocacy as neededMaintains positive working relationships within the department, with UWMC staff, and with community providers by effective oral and written communicationParticipates in quality improvement activities that enhance the performance of the department and the UWMC, as appropriateProvides education to other health care providers and staff, patients and their support systems on psychosocial issues, community resources, and the role of social workResponds to crisis situations and emergency codes, as appropriate, and participates in implementation of departmental and UWMC disaster plans in the event of internal and external disastersProvides consultation and cross-coverage as needed to other members of the departmentDocuments pertinent clinical activity and statistical data in a timely manner, to meet professional standards, departmental and UWMC policies and regulatory requirementsThis position may be hired as a Social Worker 1 or 2, depending on candidate's qualifications.REQUIREMENTS:SOCIAL WORKER 1:A Master of Social Work Degree from a program accredited by the Council of Social Work Education AND six months of experience in a health care delivery system. OR Equivalent education/experience. Health care experience can include a practicum placement as part of a social work training program and/or employment experience. SW1 LEGAL REQUIREMENT:Within 90 days of hire must hold one of the following credentials issued by the state of Washington: Agency Affiliated Counselor Licensed Social Work Associate -Advanced Licensed Social Work Associate - Independent Clinical SOCIAL WORKER 2:A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND two years of full-time professional social work experience in a health-care setting eligible to begin social work practicum instruction for the School of Social Work.
