The University of Maryland, Baltimore School of Nursing (UMSON) is recruiting for a part-time temporary Health Care Provider/Nurse Practitioner . The selected candidate will practice 1-2 days per week including providing primary and secondary specialty care in an ambulatory care based practice setting at Prince Georges Hospital Center's (PGHC) Ambulatory Care facility.

Essential Functions: Collaborate with the health care team to direct the day-to-day clinical activities and providing primary care services in collaboration with an interprofessional team comprised of pharmacist, social workers, lawyers and with other health professionals.
Ensure the availability and accessibility of clinical services to patients referred to the Interprofessional Transition of Care ITOC clinic at PGHC.
Provide clinical assessment, diagnosis, treatment, and management of chronic, stable morbid and acute common short-term illnesses.
Provide adult interventions including physical examinations, routine screenings per the EPSDT guidelines, medical interventions.
Order diagnostic interventions as necessary for diagnosis and management of clinical conditions. Interpret results and design and implement interventions to affect appropriate and optimal care and attain improved health outcomes.
Make referrals to other providers and specialists, such as Social Workers, Physicians, and Specialists when necessary for screening procedures, diagnostic consultation, or treatment.
Facilitate the transfer of health knowledge within a community through continuous teaching and counseling of patients. Foster team participation in individual and group health promotion and health education activities.
Participate in the formulation, implementation, and coordination of standards, policies, and procedures for the health services provided by the ITOC.
Collaborate with pharmacists, social workers, Wellmobile Nurse Practitioners, Nurse Coordinator, outreach worker and lawyers in designing an effective interprofessional care plan that supports optimal health and medical management of this comorbid population of high utilizers. The ITOC and the Wellmobile will work in concert to address health needs and social determinates of health in this complex patient population.
Participate in research and evaluation activities at the request of the Community Health Resource Commission Grant under the direction of Associate Dean for Pharmacy Practice.
Participate in quality assurance and peer review activities for continual monitoring and improvement in clinical services.
Precept entry-level and graduate community health in care management and graduate NP students in all areas of NP functioning in the ITOC.
Act as a liaison between representatives of the communities served and the University of Maryland; assists with the management of change within the program whenever necessary.
Provide feedback on a continuing basis to other team members and other staff on their performance relative to clinical management and interprofessional team performance.

-Completion of an accredited program and current licensure as a Nurse Practitioner. Doctorate in Nursing Practice (DNP) Degree preferred.
-Requires certification by the American Academy of Nurse Practitioners Certification Program.
-May require training related to occupational safety and health, environmental compliance, shipping of hazardous materials and/or ionizing radiation as well as electronic health record.
