DUTIES: The TH Pharmacist will perform, but not limited to, the following: - Dispenses, compounds, admixes drugs and other pharmaceuticals. - Reviews medication orders for safety and efficacy and then approves orders within our CPOE system to maintain med profiles. - Reviews and approves Pyxis profile orders. Admixes oncology, TPN, and investigational drug solutions as needed. - Dispenses controlled substances as needed and maintain files. - Participates in drug utilization review studies, QA and PI programs. - Actively participates in new technologies and operational programs. - Maintains the records of Pharmacy. - Supervises technicians and support staff. - Reports to Senior Pharmacists and the Director and Associate Director of Pharmacy. - Provides in-service education to healthcare professionals on demand. - May rotate with medical teams and/or participate in specific medication management programs. - Holiday coverage rotation and on-call/recall rotation is required. (DUTIES AND RESPONSIBILITIES ARE NOT LIMITED TO THE ABOVE POSITION DESCRIPTION) QUALIFICATIONS: - PharmD from an accredited College of Pharmacy. - New York State Pharmacy Licensure. - Three (3) years of experience as a hospital pharmacist is preferred. - PGY-1 residency is preferred.
