Position plans, organizes and implements social work services for patients and families.
PRIMARY DUTIES AND RESPONSIBILITIES:
Provides social work services for patients and their families.
Prepares psychosocial reports and maintains necessary records.
Schedules patient appointments with clinical resources and provides telephone follow-up.
Facilitates referrals and community resources for patients/families.
Verifies insurance coverage for patients.
Answers inquiries from patients/families about community resources.
Master’s degree in Social Work required.
Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
Working knowledge of computers, verbal and written ability with organizational and interpersonal skills.
The hiring range for this position is $24.16 - $30.81 per hour.
