Rutgers, The State University of New Jersey, is seeking a Social Worker I, for the Eric B. Chandler Health Center at the Rutgers Robert Wood Johnson Medical School. The primary purpose of the Social Worker I is providing social work and behavioral health consultation utilizing a team-based model to children, adolescents, adults and families in order to improve psychosocial functioning. Among the key duties of this position are the following:
Provides comprehensive assessment and diagnosis of behavioral health clients and social work support to all health center patients.
Provides effective treatment planning and assists clients in successfully achieving goals.
Evaluates crisis situations and apply appropriate interventions.
Actively participate in team and other meetings that support EBCHC's integrated health care model to provide comprehensive care for clients.
