Bachelors Social Workers (BSWs) of the Guest Assistance Program (GAP) assume leadership for educating, coordinating and assisting patients and families with identifying appropriate resources for health care needs. They provide information, education, and support to social work colleagues and clinic staff regarding available assistance opportunities as needed. The outcome of these services is to support patients and families, to help maximize their participation in and benefit from health care, and to assist in removing barriers to continuity of care. This classification requires registration, certification or licensure with the appropriate agencies as identified on the Primary Source Verification Matrix. Under FLSA, incumbents in this position are nonexempt.
These two (2) full-time 40 hour per week GAP BSW positions will be embedded in the HIV and AIDS Treatment Program (HATP), and these BSW's will work exclusively with the patients and families of HATP. The positions will require the ability to identify and assess the psychosocial needs of adults between the ages of 18 years through 100 years, including issues related to illness and hospitalization, mental health and substance abuse, including tobacco use and cessation counseling, growth and development needs, and community resources available to and used by adults and their families. The positions will require the ability to assess and provide Medical Case Management to adult People with HIV/AIDS (PWHA) around issues unique to these patients with HIV/AIDS and their families and supports. The positions will require experience with, understanding of, and expertise with biopsychosocial issues unique to PWHA; service delivery and strategies with PWHA’s; local, regional and national networks of HIV and AIDS-related services and organizations; HIV/AIDS support programs; health insurance issues; and administrative requirements of Ryan White Care Act and other grants.
Provide specialized case management practice and other medical social work services to outpatients with HIV/AIDS.
Identify patients who have a need for outreach and intensive Case Management services through HATP staff, attendance at HIV/AIDS rounds and conferences, chart review, and referral from other services. Complete assessments of need for social work services on all patients referred or identified. Assist with identifying and overcoming barriers to medical care, including, but not limited to: transportation, insurance concerns, childcare needs, etc. Identify and refer patients for issues related to mental health and substance abuse.
Provide assistance to referred or identified HIV+ individuals and/or groups of these patients in issues that affect their health care. This may include, but is not limited to:
Education with patients/families/supports who are experiencing difficulty with adjustment, coping, anxiety, depression, and grief issues; education with non-adherent patients with the goal of achieving desired treatment outcomes and enlisting improved adherence and adherence; provision of risk reduction education and counseling; tobacco cessation counseling with patients who have indicated a desire to quit smoking; community referrals; completion of applications and paperwork as related to assistance programs, such as Michigan Drug Assistance Program (MIDAP), Ryan White, Premium Assistance, marketplace insurance, Medicaid, etc.; assistance with obtaining tangible needs of patients such as parking, transportation or food; and information and education on DPOA and Advance Directives as requested.
Complete data collection and entry into both Michigan Medicine electronic medical records and CareWare.
Advocate as needed within the hospital and community on behalf of patients/families/supports to maximize access to appropriate and necessary services to enhance health care. Work closely with non-medical case management and the HATP staff to coordinate care.
Perform other related duties as assigned.
General supervision is received from a designated official.
This position requires infrequent standing/walking, as well as lifting, positioning, pushing, and/or transferring of patients.

Bachelor's degree in social work is necessary. Some experience in social work is necessary.
Current license to practice social work in the State of Michigan (LBSW or LLBSW) is necessary. This classification requires Primary Source Verification.
Complete certification for HIV Medical Case Manager, and certification for Trained Tobacco Cessation counselor, including all required trainings as directed by HATP administrator.
Follow all standards of Care for HIV Medical Case Management as directed by the Ryan White Care Coordination Grant.
Knowledge of health care systems

Demonstrated interpersonal skills and ability to form effective therapeutic relationships

This position is grant-funded through Care Coordination Ryan White funding from the Michigan Department of Community Health. Employment is dependent on re-allocation of grant.
