Social Worker or Senior Social Worker - (180001KJ)
The University of Maryland, Baltimore School of Social Work has an opportunity for a Social Worker or Senior Social Worker. This position is responsible for providing clinical social work services to the Mobile Integrated Health Program, a partnership between the University of Maryland Medical System and the Baltimore City Fire Department. The position provides social services to apply the theories, knowledge, procedures, methods, and ethics derived from a formal educational program in social work to restore or enhance social functioning of individuals and family groups through assessment, planning, intervention, case management, evaluation, and advocacy to maximize well-being. Potential to supervise social work interns, students, or employees. Provide consultation and training to other team members including Community Health Workers on topics such as patient engagement, patient centered care, motivational interviewing, etc. as requested. Service delivery engages patients telephonically and in the hospital with follow-up visits in the patient's home and/or other locations in the community. The selected candidate's position will be based on education, licensure, and experience.
The School of Social Work is one of seven professional schools at the University of Maryland, Baltimore (UMB), an urban health, law and human services campus. The School ranks among the top twenty schools of social work in the nation and has an enrollment of approximately 950 MSW students in two locations (Baltimore and Shady Grove) and about 60 PhD students are also enrolled. We also host two national training and technical assistance centers, a Child Welfare Academy, a Center on Positive Schools, and about $35,000,000 in extramural projects, in all. At this time UM SSW has about 300 employees.

Provides clinical and task supervision for social work interns, students, or employees.
Participate in care coordination as member of a team of health professionals supporting the needs of patients.
Employ a social work intake process that quickly identifies major social and behavioral threats to health and communicates that information to case management team and primary care practitioners.
Assess impact of social determinants of health on patient well-being and effectively refer patients to community resources, as needed and/or as requested by the patient. Advocate, when appropriate, on individual and system's level for change.
Engage and motivate patients to develop treatment options/goals that are congruent with identified psychosocial needs in collaboration with patient's health care team.
Provide evidence-based health coaching and brief therapy models to promote physical and mental wellness.
Communicate information regarding contact with patient to the health care team through documentation system and team meetings.
Conduct home visits as needed to support patient well-being and plan of care goals.
Complete all documentation and administrative requirements including timely entry into the medical record.
Assist in development of policies, protocols, or procedures as requested.
Performs other duties as assigned.
Social Worker : Masters in Social Work. Licensed as Masters Social Work - (LMSW) in the State of Maryland. One (1) year post-licensure (LMSW) experience in a health, behavioral health, or integrated behavioral health setting is Preferred. Senior Social Worker : Masters in Social Work. Licensed as Certified Social Work (LCSW) in the State of Maryland. One (1) year post-licensure (LCSW) experience in a health or behavioral health setting including responsibility for training, work coordination, and monitoring the work of others.
Knowledge, Skills, and Abilities:
Knowledge of best practices in the field as well as relevant federal, state, and local rules, regulations, and programs/policies utilizing proper application of diagnostic tools
Knowledge of mental health, health and substance use problems.
Skilled in screening, assessment, brief interventions and referral to treatment.
Strong interpersonal skills; ability to effectively interact with a diverse client population using various approved interviewing techniques and consultative approaches
Ability to continuously seek improvements in order to support the goals and strategic mission of the program.
Ability to use a consultative approach to resolve issues in area of responsibility.
Ability to use measurable and verifiable information for resolving problems and facilitating decision-making processes.
Excellent organizational and project management skills. Ability to manage situations with tact, diplomacy, understanding, and fairness. Ability to maintain confidentiality of clients and information obtained during treatment.
Demonstrated leadership ability and effective oral and written communication skills
Ability to maintain confidentiality of client information in accordance with professional standards, HIPAA (Health Information Portability & Accountability Act) and FERPA (Family Education Rights and Privacy Act) and state regulations.
Ability to independently review and authorize documentation of others for reimbursement or grant submissions.
Ability to work as part of a team, as well as with individuals and/or families from diverse ethnic, racial, and socio-economic backgrounds.
Skill in social work supervision.
Proficient in information technology word processing, spreadsheets, and databases.
Hiring Range will depend on candidate's title and will be commensurate with education and experience.
