Responsible to the Health Center Director for serving as a LCSW in the College of Nursing-Practice; interacting with patients as part of the primary care team; providing direct services to patients, families and groups; providing individual and family counseling and engages in crisis intervention in a primary care setting; working to address other social needs of patients and will utilize area referral sources for needed specialty care, hospitalization, or advance psychiatric assessments and treatments; identifying opportunities for developing integrated Behavioral Health Services in the primary care setting and serves as a resource to the provider staff; experience working with an interdisciplinary team and the low income population with a non-judgmental attitude; able to complete brief intervention assessments; and performing other related duties as assigned.
