Hunterdon Healthcare System comprises a 178 bed, Magnet designated, non-profit community teaching institution and 29 outpatient practice sites in New Jersey. The faculty member would be expected to work on a multi-disciplinary team across our healthcare system to deliver comprehensive medication management services that optimize patient outcomes. The faculty member will see patients both in collaboration with providers, and independently for chronic disease state management through collaborative practice agreements. The role of this position will also include providing population health management services, as determined by data analysis and metrics for each of our outpatient practices. The faculty member is expected to work in collaboration with the clinical pharmacy team as appropriate, and is responsible for educating pharmacists in training, pharmacy residents and other health care professionals.
