Provide short term psychosocial counseling as identified through assessment for patients and families/support systems to enhance participation and cooperation in developing a comprehensive plan. Collaborate with interdisciplinary teams to assess and plan for appropriate care and services across the continuum. Case management. Manage patient referrals, including, but not limited to, screening, identification of social work intervention needs, assessments, development of plan based on assessed needs and implementation of plans. Manage all patient satisfaction issues by leveraging resources and relationships throughout the College of Dentistry to improve patient satisfaction.
Required Education: Master degree in social work
Required Experience: 3-5 years experience of social work experience in a health care setting or social service agency
Required Skills, Knowledge and Abilities: Excellent communication (both written and verbal), presentation and analytical skills, including analysis, planning, organizing and troubleshooting. Working knowledge of patient care in an inpatient setting. Strong customer service and interpersonal skills. Ability to engage and interact with a diverse population. Ability to diffuse stressful situations and resolve issues. Current NYS licensure and board certification.
