DUTIES: The Pediatric Clinical Pharmacy Specialist will provide clinical pharmacy services to pediatric and neonatal populations at the State University of New York (SUNY) Downstate Medical Center. The Specialist will work as part of multidisciplinary teams in order to deliver safe, appropriate, comprehensive and cost-effective drug therapy. Duties include the following: Reviews patients drug therapies per direct consult or patient rounds to identify drug-related problems and makes recommendations to optimize therapy. Adjusts patients drug therapy regimen utilizing appropriate pharmacokinetic and pharmacodynamic principles. Detects and documents adverse drug reactions; Interviews and counsels patients and/or families and answers drug information questions. Provides discharge counseling to patients and/or families on medications in conjunction with physicians. Participates in the hospital medication safety initiatives. Coordinates patient care activities with the Pharmacy and Pediatric/Neonatology services. Promotes and demonstrates rational, cost-effective therapy for Pediatric patients. Serves as a drug information resource for physicians and other healthcare providers regarding optimal use of medications in Pediatric/Neonatal patient. Provides for and facilitates investigational drug studies for Pediatric/Neonatal services. Provides educational programs on medication use for healthcare professionals. Precepts post-graduate pharmacy practice residents and students. Serves on the Pharmacy and Therapeutics Committee and various Pediatric Performance Improvement Committees. Doctorate of Pharmacy Degree from an ACPE-Accredited College of Pharmacy. Completion of PGYI Pharmacy Residency. PREFERRED: Completion of PGY2 Pharmacy Residency. Board certification in specialty.
