Harborview Medical Center has an outstanding opportunity available for a temporary/hourly, part-time, MEDICAL SOCIAL WORKER to provide direct social work and implement the full range of social work for patients. This person will provide direct clinical social work services to inpatient medical surgical units or inpatient rehabilitation including social work screening and assessment of inpatients and their families. Interventions may include crisis intervention, grief counseling, discharge planning, crime victim advocacy, greater systems intervention, family counseling, resource advocacy, assistance with systems navigation, discharge referrals. RESPONSIBILITIES:Provide social work services to patients and their families, including psychosocial assessment, domestic violence screening, child abuse and elder abuse interventions, and discharge planning for inpatients admitted to the assigned medical or surgical serviceWork with patients and support systems to plan for discharge needs, including skilled nursing facility placement and other treatment programs, insurer and financial advocacy, patients/support system education, crisis interventionMaintain effective working relationships with members of the multidisciplinary team and provide leadership in areas of discharge resources and psychosocial and family systems interventionsFacilitate family, support system and network conferences as indicatedObserve Social Work Department policies/procedures, and legal mandates in carrying out social work roleResponsibilities also include documentation and verbal communication of social work activities provided to patients and their families or support system, having knowledge of legal intervention options and processes, and of community resources that are relevant to the service population Social work interventions include crisis intervention, social work assessment, resource coordination and case managementParticipate in regular interdisciplinary staff meetings and patient care roundsHave knowledge of HMC/Social Work Department policies relevant to delivery of services to patients as well as knowledge of legal requirements for vulnerable populations, involuntary treatment for psychiatric patients and patients with chemical dependenciesHave knowledge of community resources relevant to the target populationProvide telephone information and referral to individuals and agencies calling for information or consultation Provide clinical services with respect for patient/family rights and within the Social Work Code of EthicsREQUIREMENTS:A Master of Social Work Degree from a program accredited by the Council on Social Work EducationAND One year of full-time social work experience in a health care setting or equivalent.
