The PPOC-ASAP Social Worker will be an embedded, fully integrated, ASAP-affiliated clinician delivering direct services in pediatric primary care offices and participating in office-based collaborative team rounds and multi-disciplinary care. They will report directly to the Director of Integrated Substance Use Treatment Services. The PPOC ASAP Clinical Social Worker will be responsible for:
Delivering 15 hours of direct patient care per week in the pediatric office setting, with the remaining 5 hours as needed.
Performing comprehensive evaluations to assess severity of substance use problems and disorders, psychosocial and mental health needs.
Providing individual therapy to patients of the pediatric practice.
Providing case coordination for identified patients within the pediatric practice in consultation with the Director of Integrated Substance Use Treatment Services and the ASAP clinical team.
Serving as a member of a medical home team to plan and implement patient care, assist in program development and participate in continuous quality improvement efforts as it relates to the integration of substance use treatment services into the medical home under the direction and approval of the Director of Integrated Substance Use Treatment Services, ASAP, the PPOC and practice leadership.
Facilitating communication between patient and family; education of family about substance use and substance use disorders, appropriate levels of care and treatment options.
Providing consultation to medical home team members on psychosocial and substance use issues and specific patient needs; conducts related staff and/or community education programs as assigned.
Participating in departmental meetings, committees, work groups as assigned.
To qualify, you must have:
A Master's degree in Social Work with a license for independent clinical practice (LICSW) in Massachusetts
Strong clinical skills and experience with various therapeutic modalities
Experience working with substance use disorder identification and treatment is highly preferred
Flexibility and adaptability required to work in a busy medical practice
This position will be based in Auburn, MA in a pediatric office setting
