1. Completes psychosocial assessments, mental status examinations, and diagnostic assessments of both individuals and families and translates this information into medically necessary, multidisciplinary treatment plans. 2. Provides individual and group therapy consistent with evidence-based practice protocols for primary mental health, addiction, and co-occurring populations. Communicates with family, other disciplines and community agencies, as needed to ensure collaboration in treatment. 3. Provides clinical case management services, including referrals to appropriate levels of care. 4. Provides crisis intervention and clinical triage to facilitate necessary services, such as inpatient hospitalization, when indicated. 5. Determines membership and leads and facilitates psychotherapy groups. 6. Assists in determining appropriate circumstances for reintegration of patient into home and/or community. 7. Evaluates treatment progress and presents findings and recommendations at treatment team meetings. 8. Provides vocational, transitional, social adjustment, community living, and support services to patients. 9. Completes high quality, timely charting according to agency policies and procedures and payer requirements. 10. May perform other duties as assigned.
