Summary of Essential Job Duties: The Clinical Program Coordinator is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services—from initial referral to follow-up care in Kidney Transplant. Serves as the kidney transplant coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff. Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community. Collects/documents data to meet clinical, financial, and regulatory requirements. Participates in the development of guidelines, protocols, pathways, and other performance improvement activities. Participates in research activities. Serves as a patient advocate throughout the patient care process. Represents the Medical Center at regional and national meetings/conferences. The Clinical Program Coordinator provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, The Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan. He/she maintains competency skills including the ability to provide care to special patient populations and patient with diverse cultural backgrounds. He/she can identify the ability of the patients to follow directions/instructions and give consent. He/she can identify safety concerns and institute measures to provide a safe environment; and provide appropriate assessment, intervention and evaluation of patients with consideration of developmental milestones in the patient population.
