Centrally located telesitter monitor technician responsibility:
Receives report from RN regarding patient to be monitored.
Places patient under continuous visual monitoring.
Ensures that patient information is entered and set into software correctly.
Introduces self to patient over telesitter audio upon initiation and every shift (if appropriate).
Documents in the EMR.
Records initiation of continuous visual monitoring on patient log and the reason for continuous visual monitoring.
Monitors patients and attempts to verbally redirect patient through 2-way audio.
Calls nursing staff directly if an issue arises with the patient that needs staff response.
Activates STAT Alert alarm when a patient is not following direction and/or situation is emergent.
Initiates electronic privacy curtain upon request from clinical staff.Sets timer each time this is done.Communicates with RN regarding privacy time.
Changes status of patient to 'patient out of room' when patient leaves room and follows up with expected return times.
Assists House Supervisor in the process of identifying patients that could potentially have continuous visual monitoring discontinued in the event another patient has a higher need.
Provides handoff report to oncoming telesitter monitoring staff regarding patient and their status; includes notable behaviors to be watching for with the patient, and any other notes in relation to the patient.
Notifies Clinical Supervisor/Charge Nurse if patient meets any of exclusion criteria.
Patient and Family Education:
The patient and family will be informed that telesitter visual monitoring will be utilized to ensure patient safety along with education on intervention and criteria for initiation and discontinuation.
Utilize additional materials to assist with education awareness and reminder for the patient/family including posting of designated signage in the patient room, in plain view of patient, visitors and staff, indicating that continuous visual monitoring is taking place.
Document patient education in the electronic medical record and Plan of Care.
Utilization of continuous visual monitoring shall be documented in the electronic medical record, as well as patient/family notification and education related to continuous visual monitoring shall be documented in the plan of care.
The telesitter will document observed patient behaviors, interventions and communications with the nurse.
